Lost or Stolen Passport Notification This letter should be used to follow up with the U.S. Department
of State or local passport office if your passport was lost or stolen.You should immediately report it by filing Form DS-64: Statement
Regarding Lost or Stolen Passports. You can also apply for a new passport by filing Form DS-11 Application for U.S. Passport or Registration

Theft Report Information to Police If you are the victim of theft,
the first thing you should do is contact the police. The police will request that you visit the police station so that you can be
interviewed and an official police report can be prepared. Credit card companies, banks, insurance agencies and other entities that
you might be contacting as part of the resolution process will want proof that you have been involved in a theft incident. A completed
police report is evidence that you have been the victim of theft. It demonstrates that you are serious about remedying the problem.
The information provided in this police report document is a sample so that you know what information will likely be requested by
the police. Requested information may vary among local police departments. However, gathering the information should prepare you well
for an interview with the police. Offer to provide the police with a signed copy of this document. The police may wish to attach the

agency who has either failed to respond or denied some part of your previous request for information under the Freedom of Information
Act To request that the search and copying fees be waived or reduced due to an exemption
Medicare Review, Complaints, or Appeals The
Complaint letter is used to make a written complaint to a Medicare headquarters regarding a service received under Medicare (other
then Peer Review issues - see later in paragraph) or to resolve a problem regarding a Medicare service that has not
been resolved at the local level. The Appeals letters are used to request that Medicare reconsider its initial decision on a
Part A claim or to Request a review of a Part B denial. The Review letter is used to allow a Medicare recipient to advise the
Peer Review Organization (PRO) about the quality of care received during a hospital stay or to request a review of a written Notice
of Noncoverage that Medicare will no longer pay for hospital care To request a review of a "Notice of Noncoverage" that you have
received To advise the PRO about the quality of care received
Response to IRS Notice or Penalty One letter is
used to respond to an IRS notice of a proposed adjustment to a federal income tax return You have received a notice from the IRS
proposing an adjustment to your return because an item of income was not reported You disagree with the proposed adjustment to your
return.The second type of letter requests a review of an IRS penalty assessment You have been assessed an IRS penalty and would
like the penalty reduced or eliminated You have reasonable cause to request a review of the penalty assessment. If you used a Tax
Professional or Tax software to prepare the return in question, you should consult them as to how the error, omssion, or penalty
arose and whether they can correct or reimburse you for the error or omission
Letter of Complaint about Federal, State or Local
